For all those wondering why the USA has been disqualified, they had violated technical rule 24.19.
According to it,"For all takeovers, athletes are not permitted to begin running outside their takeover zones and shall start within the zone. If an athlete does not follow this Rule, their team shall be disqualified.

Confusion over fourth runner
There is some confusion over the fourth runner for the event. While a list by the Athletics Federation of India (AFI) shows Sarthak Bhambri as their fourth runner, the Olympic website has Rajiv Arokia in the heats list.
The confusion is expected to clear soon. In all likeability, it will be Arokia taking to the track.
