On October 20, 2015, two police officers drove by Barker's house in the Los Angeles area and saw him tripping on a sidewalk. They called the ambulance and took him to Cedars-Sinai Medical Center. There he got stitches on his forehead for an injury. His left knee was also hurt.

On June 19, 2017, Barker slid and struck his head once again in his home. His housekeeper took him to the E.R., where he was examined and released. His rep stated that this was not as severe as his previous fall. His biggest challenge was in 2018 when he was hospitalized twice at the end of the year for severe back pain which led him to discreetly celebrate his 95th birthday at that time.

His mother, Matilda Valandra, was a school teacher and his father, Byron John Barker, was the foreman of the high power line across Washington State. Barker's father was a quarter Sioux (Native Americans) and his mother was not, so Barker was the eighth Sioux. Barker attended Mission Sioux School. Barker spent much of his early life on the Rosebud Indian Reserve at Mission, South Dakota.

In World War II, Barker trained as a fighter pilot in the American navy. Having graduated from Drury College in Springfield, Mo., he focused on a career in radio and then worked at a station in California. Barker started hosting the game show Truth or Consequences, where candidates who did not properly answer trivial questions had to perform stunts, in 1956. From the show, his kindness quickly got in touch with the audience. He was on the show until 1975. During his time on Truth or Consequences, Barker also hosted a number of short television games.
