In the opening scenes of The Last of Us Part 1, players get introduced to Joel's daughter. As the story unfolds, players tune into updates through news broadcasts, getting the lowdown on failed vaccine attempts and the Army imposing martial law in big U.S. cities. This sparks the rise of the Fireflies, a group pushing for the return of democracy. Fast forward two decades since Sarah's passing, and Joel is navigating life as a smuggler in a heavily guarded Boston.

As the story unfolds in the present scene, Tess, Joel's partner in crime, knocks on his door with urgency. She reveals that a man named Robert has sent a couple of thugs after her. Joel and Tess decide to confront Robert, only to discover that he has betrayed them by selling their guns to the Fireflies. In a tense confrontation, Robert meets his demise.

Following this, a Firefly named Marlene presents Joel and Tess with a proposition: if they agree to smuggle a young girl out of Boston, they'll get their weapons back. This unexpected turn in the narrative sets the stage for the beginning of a truly remarkable relationship, making it a pivotal moment in gaming history.

It was a momentous occasion when Joel crossed paths with Ellie for the first time. As Joel and Tess spent time with her, they had a surprising revelation: Ellie possessed a natural immunity to the devastating cordyceps virus that had wreaked havoc on civilization.

With a deal in place, they ventured out of the militarized zone. Joel grew fond of Ellie as she reminded him of his daughter Sarah. However, two decades of ruthlessly dealing with threats, infected or not, had left Joel with a hardened heart, making it challenging for him to open up emotionally to others.

Hoping that Tommy could connect them with other Fireflies, Joel and Ellie embark on a journey to the west side. The challenge is that Tommy resides far from their current location, and they lack the means to reach him. Joel suggests meeting an old buddy of his, a quirky loner named Bill, who lives in the suburbs outside Boston.

Eventually, they locate Bill, a grizzled individual who has been left by his partner and dealt with the tragic aftermath of their suicide. Bill, clearly marked by nihilism and concealing the pain of solitude, advises Joel against helping Ellie. Nevertheless, honoring a debt to Bill, Joel secures his assistance in obtaining a car for their journey. With the vehicle in hand, the duo sets out on the road, driving west in search of Tommy.

As Joel and Ellie head towards Pittsburgh, they encounter a surprise attack from an outlaw gang. In the middle of fighting for their lives with a group of bandits, there is the most intense moment; Joel’s life hangs in the balance by a thread, and it is Ellie who courageously steps into action to save his life.

As time passes, Joel and Ellie eventually make it to Jackson, where Tommy resides in a community. Tommy warmly greets Joel, inviting him and Ellie to become part of their group. Joel shares the news of Ellie's immunity with Tommy, explaining his plan to deliver her to the Fireflies for vaccine development.

Tommy generously offers Joel some gear for their journey but respectfully declines taking Ellie away from him. However, when Ellie discovers that Joel considered handing her over, she reacts strongly, feeling betrayed by a man she's come to regard as a father figure.

In the midst of a heated argument, Ellie brings up Joel's late daughter, Sarah, hurting him deeply. Joel, in frustration, clarifies that Ellie is not his daughter and insists they part ways. After some arguments, Joel and Ellie embark on a horseback journey to Colorado, where Tommy believes they might find some Fireflies.

Fast forward to the winter, Ellie fends for both of them. While hunting for deer, she encounters a man named David, who proposes a trade of penicillin for deer meat. As they form an unexpected connection and face infected threats together, David discloses that his gang was the one that attacked Joel and Ellie. He tries to win Ellie's trust, but things take a dark turn when, the next day, David's men capture Ellie and imprison her.

Joel and Ellie finally reach Salt Lake City after a difficult journey. There, they have an emotional talk, and Ellie shows Joel a photo of him with Sarah. After numerous dangers of being infected and traveling through the city, Joel and Ellie finally make it back to the remaining Fireflies at St. Mary’s Hospital. This becomes quite a significant point in The Last of Us 2.

In the hospital, Joel reconnects with Marlene, who discloses that Ellie is being prepared for surgery. The Fireflies aim to create a vaccine that could save humanity, but the grim reality is that Ellie won't survive the procedure. Filled with anger and frustration, Joel confronts Marlene over the decision to sacrifice Ellie, leading to his escape from Firefly custody.

In a desperate attempt to save Ellie, Joel storms into the operating room, where a surgeon tries to reason with him about the potential lives they could save. Joel, however, takes matters into his own hands, shooting the surgeon and rescuing the unconscious Ellie.

As he flees the hospital, Marlene tries to stop him, emphasizing that Ellie should have a say in sacrificing herself. Despite her pleas, Joel, forced to make a difficult choice, kills Marlene and escapes with Ellie.

On their way out of Salt Lake City, Ellie regains consciousness and questions Joel about the incident. To protect her from the harsh truth, Joel fabricates a story, claiming that the Fireflies have abandoned their vaccine efforts after finding other immune individuals. Making a promise to Ellie, Joel assures her that everything he said about the vaccine is true, concluding the first chapter of The Last of Us while setting the stage for The Last of Us 2.
