Pedro Pascal is Joel, a middle-aged survivor who is haunted by his past traumas and is tasked with escorting a young girl named Ellie across the United States. Unlike in the game, Joel is portrayed as more vulnerable in the series, dealing with hearing loss and physical pain. Pascal was chosen for his ability to portray a complex and emotive character and was attracted to the project due to his desire to work with co-creator Craig Mazin. He is one of the highest-paid actors on American television, earning $600,000 per episode.

Bella Ramsey stars as Ellie, a 14-year-old girl who is immune to the Cordyceps infection and may hold the key to creating a vaccine. Like in the game, Ellie is a lesbian. Ramsey was chosen for her ability to portray a resourceful and nuanced character and was encouraged to not play the game in order to bring a fresh interpretation to the role. She learned an American accent for the role and had to cut off a significant amount of her hair.
