While the match at Wrestlemania 38 will certainly feature at least one title, it doesn't need titles to be told. Reigns and Lesnar have been rivals for years, dating back to 2015. Since then, the two have faced each other several times over titles.
Reigns has dethroned the Beast while the Beast has also put back challenges from the Big Dog. The main wrinkle this time is that Reigns is a heel, and Lesnar is the face.

The feud is also about Paul Heyman's involvement with both men. He has aligned himself with both men throughout the angle but turned on Lesnar at the Rumble.
For that reason, there is enough for a good story to be told. Heyman's betrayal was a good twist that added more to the story. The angle was good enough already without the need to add another Championship to fight for at WrestleMania 38.
