Shinsuke Nakamura and Jeff Hardy finally squared off against each other after a month's buildup of the feud in the 2018 edition of Extreme Rules. The two had been supposed to face each other earlier, but a dog bite threw a wrench into those plans.

Nakamura and Hardy faced each other, with Hardy putting his United States Title on the line. Nakamura had come in with a plan, and when the referee had his back turned, he hit Hardy with a low blow. Hardy struggled to stand up again, while the referee asked him if he could continue. Hardy agreed to go on with the bout, and the referee rang the bell. Immediately upon the bell being rung, Nakamura ran up to Hardy and hit him with a Kinshasa. He then went for the pin without delaying, thereby picking up the win within 5 seconds of the match starting.

Nakamura became the United States Champion, leaving the writhing Hardy to look on as he celebrated. His celebration was short-lived, however, as Randy Orton interrupted him. He came down to the ring, and after a staredown with Nakamura, kicked Hardy in the crotch without any warning, leaving the audience wondering what his motive could have been.
