The episode begins with a flashback of young Yhwach crying as he witnesses his father, the Soul King, sacrificing himself to become the lynchpin of the three realms. Interestingly, Ichigo Kurosaki confronts those visions as he heads to the Soul King's palace.
Meanwhile, Yhwach stabbing the Soul King sends tremors across the three realms. Just then, Ichigo arrives at the Soul Palace's main entrance and plans to stop Yhwach. The episode then highlights Ichigo Kurosaki's battle against Yhwach, which takes them to multiple locations in the Soul Palace.
Ichigo demonstrates Getsuga Jujisho at Yhwach, but the Quincy King appears unfazed. Although he acknowledges Ichigo's strength, Yhwach feels it's not enough to stop him because he has yet to use his Almighty eyes. As soon as the Quincy King awakens his Almighty eyes, Ichigo's attacks no longer land a hit.
Meanwhile, Renji, Rukia, and others at the Seireitei decide to go to the lab and ask Kisuke Urahara about the reason behind the tremors. On the other hand, Nanao wonders why the Quincies have gone to the Royal Castle instead of wiping out the Soul Reapers.
Just then, the episode shifts to Yhwach, who laments that the Soul King (whose name is Adyneus) foresaw the Quincy's tragedy in the future and yet chose to abandon them. That's why, he desires to eliminate the Soul King and create a new world. Ichigo Kurosaki regains his stance and tries to stop Yhwach.
However, the Quincy King's abilities prove too difficult for them to handle. At that moment, Ichigo notices that the Soul King has been stabbed. He charges past Yhwach to save the world's keystone. Yet, as soon as Ichigo pulls out the sword, his Quincy Blood interferes with his limb movements, causing him to slice the Soul King in half.
Ichigo becomes crestfallen, as Yhwach reveals how he planned the whole thing from the get-go. Moreover, it is revealed that only Ichigo, who possesses the powers of five beings, i.e., the Soul Reapers, Humans, Quincies, Hollows, and the Fullbringers, could be used to kill the Soul King. And the inherent hatred for the Soul King in his Quincy part pushed him for the kill.
Meanwhile, the Soul King's death sends even more tremors throughout the Seireitei, Hueco Mundo, and the World of the Living. Kisuke Urahara feels that at this rate, every realm will cease to exist. The episode ends with Yhwach telling Ichigo to witness the Soul Society's destruction.

According to the title preview shown at the end of the latest installment, Bleach TYBW part 3 episode 3 is titled The Dark Arm. The episode will likely cover the events from chapter 615 onwards and show Jushiro Ukitake making the ultimate choice to save the realms from falling apart.
The episode will reveal the actual secret behind the Kamikake ritual, something Jushiro has already done. This ritual could likely prove useful in saving the Reio or at least stabilizing the current situation.
