There's no specific procedure to follow to obtain the free rewards. Simply update the game, launch it, and the items will be added to your in-game inventory. Here are all the year-wise rewards:

Year 1 reward: Black Ice skin for Ash and Iana’s G36C.
Year 2 reward: Operation Health 2017 Card Background.
Year 3 reward: Decontaminated 2018 Headgear for Sentry.
Year 4 reward: Sleeptight 2018 skin for Kali’s CSRX 300.
Year 5 reward: Stand and Deliver 2020 Tachanka Operator Portrait.
Year 6 reward: Ye Olde Boom 2021 skin for GONNE-6.
