The soundtrack for Ballerina was shaped through collaborations involving director Len Wiseman, composers Tyler Bates and Joel J. Richard, and music supervisor Rachel Levy. According to Film Music Reporter in a report published on May 29, 2025, the aim was to align sound design with the tone of the John Wick franchise while allowing for stylistic additions unique to the character of Eve Macarro.

Bates and Richard composed music that blends high-intensity electronic themes with orchestral elements to complement the film's choreography-driven action sequences.

Before the final composers were selected, Marco Beltrami and Anna Drubich were attached to the project. Beltrami had previously collaborated with Wiseman on Underworld: Evolution and Live Free or Die Hard. However, by September 2024, Bates and Richard were brought in to replace them as the film's musical direction shifted closer to the established sound of previous John Wick installments.

In addition to the instrumental score, Columbia Records released the digital single Hand That Feeds, performed by Halsey and Amy Lee, on May 9, 2025. The collaboration brought together pop and rock influences to reflect the duality in Eve’s journey. Another featured track, Fight Like A Girl by Evanescence featuring K.Flay, plays during a critical fight sequence and ties into the film’s themes of resilience and identity.
