Rey Mysterio got into a rivalry with Big Show, soon after WrestleMania 19. The behemoth made quick work of Mysterio at Backlash 2003, and pinned him after hitting a thunderous Chokeslam. As his music hit, Big Show made his way to the back, but the worse was yet to come. Mysterio was placed and tied on a stretcher by a bunch of medical personnel to keep his body steady.

Suddenly, Big Show appeared from behind and swatted two personnel on both sides. He picked up the stretcher with Mysterio bound to it, and hit it with full force on one of the ring posts. As soon as the stretcher hit the post, Big Show let it go, and it fell down with Mysterio's face hitting the floor. The brutal attack was enough to keep Mysterio out of action for around a month. He came back on the Blue brand in May, defeating Crash Holly and Shannon Moore in a Tripe Threat Elimination match.
